We are recruiting for an experienced BSW to fill a full-time position for fast-paced sub-acute and long-term units in our beautiful facility.

Responsibilities and Qualifications Include:

• Discharge planning for the elderly
• Completion of MDS
• Strong knowledge of community resources
• Excellent organization, interpersonal and computer skills are required.
• Long-term care experience preferred.
• Customer service focus
• Ability to contribute well to an interdisciplinary team
• Bachelors degree in a related field is required, BSW preferred.

Excellent compensation and benefits package. Compensation is negotiable based on experience.
