Job description
Adult Day Health Care center (Community-Based Adult Services program) hires Social Workers.
Social Worker duties include:
- Assessment of each participant
- Creating an individual plan of care for each participant
- Providing counseling, individual and group interventions according to the plan of care
- Providing referrals to available community resources
- Serving as a liaison for each participant
- Maintaining social service records for each participant
Position qualification:
- Master’s degree in social work from an accredited university
- Bachelor’s degree in social work from an accredited university (qualifies for assistant social worker position)
- CPR and First Aid certificate
Full time, part time, on-call options are available. Bilingual (Russian, Ukrainian) is preferred.
Job Types: Full-time, Part-time
Pay: $20.00 - $25.00 per hour
