JOB TITLE: SOAR Benefits Specialist

EMPLOYMENT TYPE: Full-time, 40 hours/week

SCHEDULED HOURS: Flexible, M-F hours between 9am-5:30pm

PROGRAM/LOCATION: SOAR, Manchester CT

OBJECTIVE OF POSITION:

The SOAR Benefits specialist will use the SSI/SSDI Outreach, Access, and Recovery (SOAR) model in order to assist individuals experiencing or at risk of homelessness who have a serious mental illness, medical impairment, and/or a co-occurring substance use disorder apply for Social Security disability benefits.

DUTIES & RESPONSIBILITIES:

  • Work with referral sources and community partners to identify applicants through team meetings, outreach, and referrals
  • Complete interviews with individuals to gather information to complete SSI/SSDI applications
  • Gather medical records and other information to complete SSI/SSDI applications
  • Write SOAR Medical Summary Reports for individual applications
  • Accompany individuals to appointments at the Social Security Administration
  • Coordinate visits to medical doctors, psychiatrists, and other specialists to obtain evidence for the application
  • Coordinate case management services with partners and help with providing case management services to individuals when needed
  • Assist the team with administrative tasks as needed

QUALIFICATIONS:

Education: Bachelor’s Degree required, concentration in Human Services preferred.

Experience: 2 years’ experience working with homeless population preferred. Knowledge of medical and psychiatric terminology and ability to synthesize medical records into a summary.

Licensure/Certification/Registration: Valid driver’s license.

Training: Must Complete SOAR training within two months of hire.

COMPENSATION RANGE*:

$20.65/hr min - $30.98/hr max

  • Actual rates are determined at the time of offer and are based on relevant experience, as well as, adherence to our internal equity policy. Many of our positions offer program/location differentials and/or sign-on incentives. (Please note, for part-time positions this range would be pro-rated based on hours worked)
