Roadmap To Peace (RTP) at BACR is looking for a bilingual Senior Case Manager who has experience serving the following priority populations: youth and young adults between the ages of 13-24 years old, justice-involved, undocumented, disconnected from education, unemployed and underemployed, public housing residents of San Francisco, pregnant and parenting. All applicants should be able to act with a sense of urgency, carry an intensive caseload of up to 15 clients, should be confident with problem-solving skills, and be innovative in working with multi-barrier youth.

Requirements

Minimum Requirements:

  • Must be fully bilingual (English/Spanish), both verbal and written
  • Must have 4+ years of case management experience
  • Must have 4+ years of experience working with court-involved youth, juvenile and adult probation departments and corresponding court systems, undocumented youth, and disconnected high school and transitional-aged youth
  • Ability to work full time (Mon-Fri, possible to work some evenings and periodic Saturdays)
  • Must have a valid CA driver’s license and reliable vehicle for work purposes
  • Must be familiar with the framework or practice of Youth Development
  • Must be familiar with the concept and practice of Restorative Justice
  • Must have knowledge of the issues plaguing bay area children and youth to better supervise and support the diverse and often high-risk population RTP serves

Responsibilities:

  • Represent BACR at internal/external networking and partnership meetings/events; point person for all case management-related projects on behalf of BACR
  • Provide program updates and progress reports on a bi-weekly basis, unless otherwise requested
  • Develop and maintain communication and positive relationships with organizations, institutes, community partners, and employers
  • Conduct various assessments of each youth; assess them in the areas of risk, academics, workplace skills, and/or behavior; identify barriers, strengths and individual characteristics
  • Work with youth in setting educational, professional and behavioral short and long term goals
  • Create stabilization plans to meet and address barriers using internally developed case management tools
  • Meet with youth in person weekly, depending on need and risk assessment, to formally monitor progress of stabilization plans
  • Check in via phone call, text and email on a daily to weekly basis for informal monitoring
  • Responsible for maintaining case notes on each client; accurately and regularly input data into database system, spreadsheets and funder databases to maintain participant information in accordance with BACR’s standards and any program-specific funder(s)’ requirements
  • Make referrals to other services and programs as appropriate; attend and/or bring clients to such activities when necessary. Referrals and linkage will meet educational and vocational, substance abuse treatment and other rehabilitative programs to address criminogenic needs
  • Build relationships and maintain regular contact with parents/guardians, foster parents, teachers/school administrators, therapists, social workers, attorneys, probation officers and other key stakeholders
  • Attend school meetings, court hearings and other appointments influencing the clients; act as advocate when appropriate
  • Responsible for writing character and recommendation letters for court on behalf of participants
  • Support clients’ families as appropriate; assist with strategizing with families and lawyers when clients are facing charges; provide translation at court, etc.
  • Participate in coordinating collaborative case conferencing sessions; facilitate phone and in person meetings between stakeholders that are each serving/supporting participants; collect status updates and additional data on participants from partners
  • Build partnerships and collaborations leading to better outcomes for participants
  • Work closely with academic and career coaches serving shared clients

Roadmap To Peace implements and coordinates collaborative case conferencing that is youth and young adult centered, identifying primary and secondary care providers to ensure individuals are successful in their development. Roadmap To Peace serves Latinx youth from 13-24 years old with ties to San Francisco who are violence involved and underinsured. RTP provides youth case management, connecting them to trauma recovery healing services, substance abuse treatment, academic services, workforce, medical services, legal support, tattoo removal, safe passage, and cultural awareness, as needed.
