Job Summary and Qualifications

The case management department is essential in expediting the patient’s use of services along the continuum of care. What You Will Do In This Role:

You will be responsible for coordinating the plan of care for the patient, managing their length of stay, ensuring appropriate resource management and developing a safe appropriate discharge plan in collaboration with the multidisciplinary team.

You will coordinate activities that promote quality outcomes and patient throughput while supporting a balance of optimal care and appropriate resource utilization.

You will identify cases with potential for high risk complications and act as an advocate for the individual’s health care needs.

You will, In partnership with Social Services, be responsible for ensuring the post-acute medical needs and level of care are appropriate

What Qualifications You Will Need:

  • Must have a minimum of an Associates Degree in Nursing - BSN is preferred. If hired into RN required position after September 2014 and no BSN, then BSN required within 3 years of facility date.
  • Must have a minimum of one year recent experience in Case Management (hospital experience is preferred)
  • Two years of clinical experience in an acute care setting is preferred
  • Must have a current Texas State RN license or Compact license
  • Must possess basic knowledge of Federal and State requirements set forth by regulatory agencies



This role requires you to be fully vaccinated for COVID-19 based on local, state and /or federal law or regulations (unless a medical or religious exemption is approved).
