Job description
Program Therapist
- Case Management, including treatment planning and discharge planning
- Conduct therapy groups
- Document all groups, family, individual, and communication notes within a specific time frame
- Attend weekly treatment team meetings
Licensed Practical Nurse
- Responsible for medication administration
- Medication counts and ordering
- Observes and charts patient's behaviors, mental status, and activities.
