THIS POSITION IS PART TIME TO START, AND FULL-TIME OPPORTUNITIES IN THE FUTURE AND WILL BE WORKING WITH OUR NATION'S VETERANS!

We are currently looking for experienced Registered Nurses (RN) to join our team in Livingston County! You must have a minimum of 2 years of Skilled Home Care and OASIS Visit experience.

Please Note: We are unable to consider any candidates who fail to meet this minimum requirement.

RESPONSIBILITIES:

• Conduct an initial and ongoing comprehensive assessment of the patient's needs, including Outcome and Assessment Information Set (OASIS) assessments at appropriate time points;
• Obtain a medical history from the patient;
• Evaluate the patient and home situation to determine what health teaching will be required, if additional services are needed and the frequency of this service;
• Develop and implement the nursing care plan;
• Document services and education provided to the patient, using an Electronic Medical Record (EMR) application; No Place Like Home Health. MUST be able to adhere to strict deadlines for Start of Care (SOC), revisits, re-certifications and discharges;
• Coordinate total patient care
• Participate in Case Conferences, In-Services and mandatory Staff Meetings as needed.

MINIMUM REQUIREMENTS:

• Must have a valid, unencumbered Registered Nursing License from the State of Michigan;
• Must have a minimum of two (2) years of Skilled Home Care / Visit nursing experience;
• Must have a valid Drivers License, your own reliable transportation and proof of current Auto Insurance;
• Must be willing to travel within a designated radius (from your home address); this radius will be agreed upon during the interview process and upon hire.
• Covid-19 Vaccination Required

Job Types: Full-time, Part-time

Pay: $60.00 - $110.00 per visit

Pay: $60.00 - $110.00 per hour
