Responsibilities
- Timely travel in personal vehicle to patient/s home
- Detailed and timely charting abilities
- Medication administration
- Ability to follow and perform tasks outlined in the patient/s individual care plans.
- Clear and timely communication with the patient/s care team
Qualifications
- Must have a current LPN/RN Nursing License in the State of Utah, required
