Job Purpose

The Quality Improvement Associate will be responsible for assisting with identification, implementation, and monitoring Quality Assurance (QA) and Performance Improvement (PI) metrics and activities. QA is defined as the specification of standards for quality of service and outcomes, and a process for assuring that care is maintained at acceptable levels. This includes Infection Control, Patient Satisfaction, and Patient Concerns. PI is the continuous study and improvement of processes with the intent to better services or outcomes, particularly in the context of community health and often includes focus on specific quality metrics such as diabetes or cancer screening.

Duties and Responsibilities

· Member will work closely with the Director of Quality Assurance and Performance Improvement and the PI Teams on development and implementation of PDSAs (Plan, Do, Study Act cycles) for the quality improvement initiatives chosen by engaging with the center staff, performing patient education, and patient outreach

· Assist in identifying clinical gaps of care as well as data surveillance on the population health of selected cohorts of patients and our patient population as an entire health center

· Ensure teams are aware of opportunities to improve clinical outcomes, and will track and provide feedback to the teams on health metrics

· Member will summarize data and report progress at monthly Staff PI Meetings

· Assist with chart reviews and data clean up, such as obtaining and attaching external lab and image results

· Collaborate with the Data Analyst to create and run data reports related to PI/QA

· Assist with monitoring, tracking, and analyzing results of Quality Assurance metrics including Patient Concerns, Patient Satisfaction, Incident Reports, and High Risk Procedures

· Coordinates the PCMH+ grant in collaboration with the Director of Clinical Services

· Coordinates NCQA accreditation along with CHN (Community Health Network)

  • Complies with all infection control policies and procedures outlined by the organization in accordance with professional, state, and federal guidelines.

· Performs other related duties as required

Qualifications

Abilities:

· Ability to apply and understand statistical methodologies

· Analytical thinking

· Data management (queries, running reports) and Data Analysis

· Experience with Electronic Health Records

· Experience working with Data Storage platforms

· Ability to work as part of a multidisciplinary team

· Ability to establish and maintain collegial relationships with coworkers.

· Ability to work independently with strong decision-making skills

· Interest in serving low income-multicultural populations

Preferred Experience:

· Experience in ambulatory primary care setting, preferred

· Experience with the IHI Model for Improvement (PDSA cycles, Driver diagrams, etc.)

· Demonstrates flexibility and resilience in response to changing clinical or administrative environments.

Skills:

· Effective communication skills (verbal and written)

· Listening skills

· Customer service skills

· Computer skills (primarily Excel)

· Flexibility

Education/Certifications:

· Bachelor’s Degree, preferably in or related to health science, public health, nursing, health informatics, biomedical science

· BLS Certification

Job Type: Full-time

Pay: $50,000.00 - $60,000.00 per year
