Full Time Public Health Nurse - Family Case Management
Minimum Starting Salary: $50,257
Hourly Rate: $25.7728
Position Schedule: Primarily weekdays, occasional evenings may be required

Position Summary
Under general supervision, provides professional nursing services to patients in their homes or in a clinic setting. Work involves performing public health nursing duties such as assessing the physical and mental health of patients, providing nursing services according to physicians' orders, locating the appropriate community resource, and referring the patient. Work is performed according to public health nurse training, OSHA standards, public health, DHS, state requirements, and county policies and procedures. Work is reviewed by a nursing supervisor daily by observation of services provided and review of patient and other records.
Essential Duties and Responsibilities

Travels to patient's home to conduct assessments or to provide nursing care/referrals.
Provides nursing care according to physicians" orders and department regulations either in the home or a clinic setting.
Provides patients with information on public health procedures, treatments, and other services, and refers patient to the appropriate social or medical agency.
Operates and cleans laboratory equipment used in the assessment process.
Collects and compiles needs assessment data; develops goals based on data.
Maintains patient and other records.
Performs related work as required.

Knowledge, Skills and Abilities

Knowledge of modern theory and practices in professional nursing.
Knowledge of public health theory and practices relating to nursing.
Knowledge of the physical illnesses, obvious symptoms, and appropriate medical terminology.
Knowledge of community resources providing services to public health patients.
Ability to assess a patients' physical and mental condition according to EPSDT and other standards, and take appropriate action prescribed in physician orders or standards.
Ability to keep accurate patient records and prepare reports.
Ability to respond to each patient with empathy and respect.
Ability to communicate in a friendly, pleasant, and courteous manner.
Ability to work with clients from varied socio-economic backgrounds.
Ability to read, understand, and apply state and county policies and other regulations.
Ability to establish and maintain effective working relationships with supervisor, other employees, patients, and family members.

Required Qualifications
Graduation from an approved school of nursing, included or supplemented by course work or training in community or public health.
Preferred Qualifications
Bachelor"s degree in Nursing (BSN), bilingual in Spanish or French a plus.
Physical Attributes/Demands
This position requires activities such as walking, standing or sitting for long periods of time, reaching, pulling, bending, stooping, and lifting light to medium objects.
Work Environment
The work is performed in an office environment and involves exposure to normal, everyday risks which require normal safety precautions typical of offices. Work may also be performed in a clinical setting, in patient"s homes, and outside, exposing the employee to inclement weather conditions.
Work also involves operating an automobile in a variety of weather conditions.
