Job Description:
Under the direct supervision of the TB Program Manager and the Nurse Case Managers (NCM), Public Heath Region 6/5S,provided routine public health nursing
work for the Tuberculosis (TV) and HIV/STD programs. Work involves providing TB case management and directly observed therapy (DOT)/directly observed preventative
therapy (DOPT) for he car, treatment and general welfare of patients diagnosed with TB, and the administration of medications to treat bacterial STI's including chlamydia,
gonorrhea, and syphilis within Region 6/5S county of Montgomery. In accordance with physicians orders, including through PHR 6/5Ss standing
delegation orders (SDOs), and public health Nurse Case Managers direction. administer medications or observes patients take medications, including pediatric and high-risk
clients, within Region 6/5S counties. Work includes but is not limited to observations of ingestion of medications, review of potential side effects, documentation, communications
with case managers for stable to moderate acuity patients with suspected/active TB disease or latent TB infection, and regular review of completion of treatment regiments
needed for STIs. Collects specimens and packages and ships or transports specimens to drop off locations. Performs patient management, data entry/data management into
an electronic health record and/or THISIS, and documentation of all provided services, including education. Performs contact investigations. Prepares patient summary reports
and participates in case reviews to ensure treatment guidelines are met. Conducts QA reviews of patient records throughout PHR 6/5S, assuring adherence to SDOs, timeframes
and treatment protocols to determine compliance and nursing care policies and procedures. Assists in compiling documents required to perform char audits. Monitors QA and
case management activities of other TB and HIV/STD team members. Evaluates medical chart data for compliance requirements. Conducts and/or participates in QA reviews
within the region and those conducted by the Central Office and as requested, participate in QA reviews of local health departments or other regions. Completes reports as
requested by lead program staff. Provides trainings for patients, providers, and community; promotes public health prevention/promotion within community. Provides
administrative support including but not limited to assistance with making phone calls/follow-ups, sending faxes , and scheduling patients. Abides by Regional SDOs and
Texas TB Work Plan. Works under limited supervision, with considerable latitude for the use of imitative and independent judgment. Works evenings, holidays, and weekends
as requested by the TB and HIV/STD program as needed. Travel is required; must maintain an acceptable driving record that meets the standards developed by the Texas
Department of Public Safety (DPS) and safely operate either a state-owned or personal vehicle to conduct states business in accordance with DSHS Policy OS-3901, Fleet
Operations Management and Procedures, and Texas motor vehicle, traffic, and driving licensing law.
Knowledge, Skills, Abilities:
Knowledge of professional Nursing theory and process, including cultural social, and economic forces in family, community, and group dynamics. Knowledge of Texas State
Board of Nursing Practice Rules and Regulations. Knowledge of nursing procedures and techniques, and the principles and processes used to providing appropriate patient care.
Knowledge of Public Health principles, practices, epidemiology, disease preventions, and health promotion. Knowledge of Tuberculosis disease process, treatment, management,
medications, and identifying side effects and adverse reactions. Knowledge of clinic/office operations. Knowledge and ability to use personal computers, network hardware, and
operating systems including printers, copiers, fax machines, and Microsoft Office.
Skill in using reasoning to identify problems, in considering appropriate resolution of situations. Skill in the use of a computer, applicable software, databases, and data entry
processes. Skill in medication administration and responding to potential side effects/adverse reaction. Skill in performing phlebotomy, obtaining, and processing specimens,
conducting diagnostic procedures including TB skin test, administering medications and immunizations.
Ability to provide patient medial care, to prepare and maintain records, to communicate effectively, and to serve as a lead worker providing direction to others. Ability to provide
TB/clinical education to patients and their families professionally, clearly, and compassionately. Ability to effectively communicate in verbal and written format. Ability to
comprehend program goals, policies, and procedures. Ability to time manage multiple caseloads. Ability to prioritize and demonstrate leadership. Ability to maintain confidentiality.

General Qualification Guidelines, Experience and Education - Must Have
  • Licensed to practice as a Registered Nurse in the State of Texas.
  • (BSN) Bachelors Science in Nursing or higher.
- BLS - Basic Life Support.
