Job Roles

  • Complete all required documentation accurately, thoroughly, timely, and in accordance with department policies and procedures, including maintaining detailed, professional electronic documentation utilizing County approved forms and formats.
  • Participate in staff meetings, supervision, training programs, outreach, and other meetings in accordance with company and program requirements.
  • Provides community-based educational and support services to parents to assist in successful completion of child's therapy goals, including teaching skills, role modeling, and helping parents to utilize family strengths to overcome identified challenges.
  • Serves as a knowledgeable source of community resources; assesses the parent/family, determines the case needs, and assists parents in navigating/utilizing community resources.
  • Serves as an advocate and navigator for participants and family members, including accompanying parents to various school and medical appointments as needed for support.
  • Performs other duties as assigned.

Education/Certifications/Licenses/Registrations

  • 2 years of professional experience working in a mental health setting with at-risk youth preferred.
  • Ability and means to travel as needed in a timely manner within San Diego County. DRIVER REQUIREMENTS: Requires an active Class C California driver's license, proof of liability insurance at $100,000, on vehicle used. No more than 2 points in past 12 months; No suspensions in last 2 years for moving violations; No DUI, reckless or felony Driving within 5 years. No license revocation in 7 years.
  • High School diploma (or GED equivalent).
  • Personal experience as a consumer of mental health, social services, foster care, or wraparound services required. Prior personal experience being a parent/caregiver or direct participant in a mental health service program is strongly preferred.

Experience/Specialized skills (including Language)

  • Ability to maintain detailed case files and documentation of progress in a professional manner.
  • Ability to work effectively with patients from diverse social, cultural and economic groups.
  • Able to maintain good working relationships with employees, coworkers, and departments.
  • Basic computer literacy ability to comply with department needs and expectations (i.e., electronic medical record documentation, obtaining background information and reports on patients, following up on appts, etc).
  • Bilingual English-Spanish required.
  • Excellent judgment and ability to establish appropriate boundaries with clients and co-workers, and to adhere to confidentiality laws.
  • Good organizational skills and time-management skills.
  • Good written and verbal communication skills.
  • Knowledge of the community resources, mental health and social service systems in San Diego County.
  • Strong interpersonal and customer service skills.

If you are hired at Family Health Centers of San Diego, your final base salary compensation will be determined based on factors such as geographic location, jurisdictional requirements, skills, education, and/or experience. In addition to these factors – we believe in the importance of pay equity and consider internal equity of our current team members as a part of any final offer. Please keep in mind that the range mentioned above is what we reasonably expect to pay for the role. Hiring at the maximum of the range would not be typical in order to allow for future and continued salary growth.
