Education: Bachelor’s degree from an accredited program/university, preferably in one of the following areas: Social Work, Psychology, Nursing, Sociology or related field of study.

Experience: 2 years of related experience and/or training in a related Human Services field.

Supervises: Lead Case Manager and Community Health Workers.

Summary: The Program Supervisor is responsible for providing direct supervision of Lead Case Manager, Case Mangers and Community Health Workers through quality assurance and improvement functions such as home/outreach observation visits, weekly productivity meetings and chart audits.

Essential Functions:
1. Supervisor has oversight of the lead case manager and community health workers services to clients including but not limited to intake assessments, state benefit assessments/applications, goal setting, case plan development, home visits, progress monitoring via standardized testing tools, client advocacy and referrals while maintaining compliance with established state and federal standards and requirements.
2. Coordination of supplemental programs and projects such as collaborative improvement, local community coalition involvement and innovation networks including community action networks.
3. Complete accurate documentation in accordance with agency policies and procedures, contract, and regulatory requirements, standards, and program timeline requirements.
4. Review Efforts to Outcomes/Salesforce reports for compliance with program requirements.
5. Meet all pre-determined reporting deadlines required by program, state, and federal partners.
6. Meet all federal and state regulatory guidelines and standards that are applicable to this position.

Measurable Deliverables:
1. Meet with lead case managers and community health workers on a weekly basis to review their weekly progress reports and provide guidance as required.
2. Review/ approve chart audits performed by lead case manager monthly or as requested by program director.
3. Participate in conference calls with partner agencies as assigned by program director.
4. Ensure appropriate referrals from lead case manager to case managers are based on client case histories and needs.
5. Ensure lead case manager conducts a minimum of 5 home visits and 5 phone calls per week to establish rapport and ensure assessments are completed in a timely manner.
6. Conduct between 1 to 3 group health education classes including specified trainings as required by supplemental programs, which include submission of required evaluation forms and data entry into our database.
7. Utilize productivity tools to monitor productivity of lead case manager, case managers, and community health workers and provide constructive feedback.
8. Ensure lead case manager is tracking that case managers maintain client contact of 2 hours or more per month.
9. Enter information is entered in Efforts to Outcomes/Salesforce as required by agency policy and procedures.
10. Increase client’s knowledge and positive changes in attitude as measured by standardized tests.
11. Participate in scheduled meetings and team decisions and operations.
12. Participate in workshops, seminars, education programs and activities that promote professional growth and development.
13. Work evenings, weekends and holidays as needed or requested by the program director.
14. Implement BCFS safety protocols, at all times.
15. Maintain BCFS HHS professional and ethical standards of conduct outlined in BCFS employee handbook including demonstrating respect for agency staff, children, and community members and complying with required dress code, at all times.
16. Other duties as assigned by program director.

Credentials, Skills, and Abilities:
1. Maintain current licenses including any certificates required for the position including, but not limited to: CPR, and First Aid.
2. Meet all federal and state regulatory guidelines and standards applicable to this position.
3. Pass a pre-employment drug screen and random drug screens throughout employment.
4. Provide proof of work eligibility status upon request.
5. Pass a pre-employment and biennial criminal background checks.
6. Drive a vehicle and comply with safety procedures in accordance with program requirements.
7. Demonstrate the ability to:
a. Work in a fast-paced environment and maintain control and professional composure, at all times.
b. Communicate effectively in writing and verbally in English (Spanish highly preferred)
c. Organize and prioritize responsibilities and duties efficiently
d. Maintain computer literacy required to meet the responsibilities of the position
e. Maintain confidentiality in all areas of the service population and program operations.
f. Work effectively and without intensive supervision both independently and as a member of a multidisciplinary team.
