The Program Manager will provide day-to-day oversight, management, and coordination over the TACE (Tenderloin Area Center of Excellence) drop in HIV medical clinic. TACE provides wraparound medical and case management services to homeless Tenderloin residents, living with HIV, substance use and mental health issues. The Program Manager will also be responsible for supervising and supporting case managers and Program Supervisor.

Essential Functions:

• Serve as day-to-day contact person for all program staff
• Coordinate all aspects of the client drop in, art therapy program, medical clinic supportive services
• Review program goals and performance and design strategies as needed to improve service delivery and ensure deliverables are being met

• Develop and maintain protocols to ensure client engagement in medical care, including scheduled medical appointments
• Develop and maintain schedule of open access programming
• Develop and maintain schedule of case managers, including requests for home visits and outreach needs
• Attend case conferences and lead staff with task management and follow up

• Ensure progress is being accomplished toward specified program objectives
• Create, organize, and implement project timelines, calendars, and schedules
• Manage, evaluate, and hire case managers and ensure that all case management objectives are being met and are aligned with the program’s overall mission of being a medical service model
• Assist with development and tracking of client treatment plans
• Assist with other administrative duties as needed

Minimum Qualifications:

• Bachelor’s degree in health or related field. A combination of experience and education will be considered.
• At least 1 year experience working with homeless communities, persons living with HIV
• Familiarity with issues and concerns of Transgender sex workers and substance users
• Knowledge of HIV/STD transmission and prevention. Knowledge of health and social issues related to HIV transmission in high-risk populations.
• At least 1 year experience with advocacy, counseling, referrals, or outreach services in the HIV field
• Demonstrated experience facilitating community groups/meetings
• Ability to work with and maintain boundaries with clients and confidential information
• Strong written and verbal communication skills
• Strong organization skills and ability to apply new knowledge into day-to-day client work
• Proficiency in Microsoft Office Programs
• Self-directed, motivated, and flexible with the ability to work independently and as part of a team.
• Understanding of the importance of confidentiality and boundaries in prevention and education work.
• Ability to work some weekends, and to be punctual and perform tasks consistently during scheduled shifts.
• Ability to support the mission and goals of SFCHC.
• Fully vaccinated against COVID-19 and able to show proof of vaccination + Booster

Preferred Qualifications:

• Master’s Degree in health-related field (MSW, MPA)
• Knowledge and experience with the drivers and health disparities in the Transgender community, and how they impact linkage and retention to HIV care services for HIV+ Transgender individuals.
• Bilingual and bicultural understanding.
• 1 year of supervision experience with peer leaders and volunteers
• 1 year of experience with case management
• 1 year of experience with curriculum development
• Professional experience working with at-risk transgender individuals, especially those dealing with substance use and mental health issues.
• At least two years of experience with advocacy, counseling, referrals or outreach services
• Familiarity and experience with Microsoft Word and Outlook programs
• Must be able to sit, stand, and walk for extended periods of time
