Starting Pay: $20.16/hour with potential increases depending on education and experience.

ACT is a program that functions as a “hospital without walls”. To this extent the role of the ACT team is to provide services and supports to individuals most affected by mental illness, who without this service would be involved in institutional care. This support member to the ACT multidisciplinary team is responsible for organizing, coordinating and monitoring all non-clinical operations under the direction of the team leader and support staff office manager.

MAIN DUTIES AND RESPONSIBILITIES:

  • Organize and maintain ACT staff work and on-call schedules, maintain accurate phone and beeper lists, share with community services as required by the program
  • Open and close charts in the EMR when requested and provide requested assistance with the upkeep of the EMR as identified
  • Scan documents into the EMR as outlined in the agency policy and procedure expectation
  • Provide coordination and assistance and documentation as needed by the team for client medical concerns and general medical health
  • Oversee the ordering, cataloging, and storage of medications for the ACT teams
  • Maintain ACT forms; create, organize, copy, scan and reorder as needed
  • Develop databases and prompts for ACT staff to meet the ACT standards for delivery of services
  • Organize and maintain computer files and keep orientation manuals updated
  • Provide essential support to the ACT teams to ensure effective and productive agency operation
  • Communicate with case managers, nurses, doctors and pharmacies to ensure timely distribution of medications to clients
  • Assist with correspondence, dictation, and reports as needed by the ACT team
  • Attend daily team meetings and participate regarding client interactions
  • Provide support to team prescriber with keeping up to date on DA’s, AIM’s and other expectations through data tracking and reminder systems as identified
  • Coordinate appointments for ACT clients with ACT psychiatrists and Clinical Nurse Specialist
  • Act as back-up (or front person) to shift managers in routing client calls, attending to client concerns and linking to resources. Coordinate communication between clients and the treatment team in timely fashion
  • Assist front support staff as needed by agency and triage phone calls
MINIMUM EDUCATION AND EXPERIENCE:

  • High school diploma or GED and one year administrative/office support experience OR combination of equivalent experience and training
