➢ Assignments: 4 Hour Shifts / 8 Hour Shifts

➢ Patient Ratio: 1:1

➢ Facility Location: Los Gatos, CA

REQUIREMENTS

➢ Minimum Education: Diploma / GED

➢ CDSS Home Care Aide Certification

➢ 2- Dose COVID-19 Vaccine + Booster(s)

➢ TB Screening (2-Step / Chest X-Ray)

➢ Valid Driver’s License + Auto Insurance

➢ Authorized to work in the U.S.

DUTIES

➢ Maintain patient supervision and safety at all times.

➢ Maintain patient confidentiality at all times.

➢ Cognizant of patient’s mental, emotional, and physical status at all times.

➢ Document & report change of condition(s) to patient’s licensed nurse.

➢ Document & report per Caregiver Log & Report.

➢ Conduct proper transitions with colleagues (i.e., debrief the following shift)

➢ Adhere to SNF policies, procedures, & guidelines.
