Manhattan Medical Arts is expanding our outpatient practice in Forest Hill, Queens, NY. We are seeking a Full-Time Family Medicine or Internal Medicine Physician to work in a 100% outpatient care clinic. In this position, the Physician is a co-clinical lead of the integrated care team that will plan, coordinate, deliver and manage comprehensive healthcare services for patients. Ensures patient/provider continuity and access to timely and appropriate levels of care.

Duties:
- Provide complete, comprehensive, family oriented outpatient care to their assigned panel of patients through the life cycle.
- Provide preventive care, manage acute and chronic conditions, develop a plan of care and coordinated care.
- Make appropriate referrals for additional services.
- Provides consultation and/or supervision to mid-level providers.
- Communicates with internal and community staff such as care coordinators, care managers and other agency staff involved in the patient's plan of care.
- Performs other duties as assigned.
Physician Requirments and Skills:
- Must be a Board Certified Physician
- Active New York State License
- DEA license
- Great interpersonal skills
- Excellent communication skills (written and verbal)
- Strong attention to detail
- Highly organized
- Able to multitask efficiently and effectively
- Solid assessment, clinical, and documentation skills
- Patient-oriented
Position requires working some morning shifts, some evening shifts and weekend shifts.
While not required, the ability to understand and/or speak Spanish is a plus.
Compensation commensurate with experience.

Pay: $230,000.00 - $300,000.00 per year
