PRE-TRIAL SERVICES NAVIGATOR

Job Summary:

The Pretrial Services Navigator (PSN) provides field-based case management services and linkage to clients released on Pretrial Supervision through Probation. The PSN's are committed to removing the client's barriers to care by identifying critical resources for clients and helping them navigate through mental health care services, substance use treatment, legal process, social services, and other identified services.
Responsibilities:
1. Manages a caseload of approximately 25 clients referred by Probation's Pretrial Supervision Department.
2. Communicates with Pretrial Supervision regularly to provide any developing changes to client progress.
3. Transports clients to and attends court hearings with clients to provide the court with progress updates and address legal issues and other associated impairments.
4. Coordinates with Pretrial Service Assessment Unit to determine client's immediate service needs upon release from incarceration. As needed, transports clients to linked service providers.
5. Interviews clients in one-on-one appointments to gather information about strengths,resources, and needs, including but not limited to: personal concerns/needs, financial concerns/needs, employment, access to necessities such as food and clothing, current and needed housing, and physical and mental impairment(s).
6. Secures information such as medical, psychological, and social factors contributing to the client's situation and evaluates these and the client's capacities and needs.
7. Assists with applying for social services benefits such as Medi-cal, General Relief, and CalFresh.
8. Provides warm hand-off and keeps open communication between referred service providers as needed.
9. Completes individual service plan and follow-ups to determine quantity and quality of service to be provided to client and status of client's case.
10. Determines the client's eligibility for mental health service and substance use disorder treatment.
11. Completes all documentation of services in ETO program within five days.
12. Provides transportation with a company vehicle to any mental health, SUD treatment, and social services appointments to establish care.

Education and Experience:
High school diploma or equivalent.
A minimum of 3 years of non-profit or related experience is strongly preferred; ·
Must have at least a high school diploma; a Bachelor's Degree in Psychology, Social Work, Human Services or a comparable combination of education/work-related experience required;
Experience in a social service setting with working knowledge of case management systems and planning techniques;
Knowledge of mental health resources, programs, and the Continuum of Care for justice-involved persons, including Behavioral Wellness and Community based Organizations in Santa Barbara County, is preferred;
Experience wording with individuals and families with mental health and/or legal issues; Must be proficient in Microsoft Word and Excel.
Possesses a high level of tolerance and understanding for individuals who present for services with urgent multiple case management and health needs
Have or be able and willing to obtain CPR/First Aid training.
Provide proof of full COVID-19 vaccination
Have reliable transportation and:
A valid driver's license
The Ability to qualify for Good Samaritan Shelter insurance coverage

Work Environment
The employee may be in contact with individuals and families in crisis who may be ill, using alcohol and drugs, and who may not be attentive to basic personal hygiene, health and safety practices. The employee may experience a number of unpleasant sensory demands associated with the client's use of alcohol and drugs, and lack of personal hygiene. The employee must be ready to respond quickly and effectively to many types of situations, including crisis situations and potentially hostile situations. The noise level in the work environment is usually moderate in an office setting. Sometimes work may become stressful when working under pressure.
