Lakewood Health System's Community Health team is growing. Our team is looking for a qualified and adaptable Population Health and Performance Improvement Coordinator!

Population Health and Performance Improvement Coordinator Population Health is responsible for the coordination of multiple organization wide programs, functions and systems in the areas of quality improvement, clinical documentation improvement, population health data analytics, and performance improvement within Lakewood Health System. Provides coordination and guidance for designing, implementing, and supporting a robust enterprise wide population health management program. This position will collaborate with and engage senior and physician leadership, administration, professional, and other staff throughout the organization to implement and lead value-based care improvement programs and performance improvement change management to attain desired performance related to payer contracts and regulatory reporting objectives while promoting a culture consistent with the health system’s mission, core values, and standards of conduct.

This is a full-time position working 64-80 hours per pay period.

• 
  Bachelors degree in Business Administration, Project Management, Public Health, Health Care Administration or similar required.

• 1-3 years healthcare related project management / performance improvement experience.

• 1-3 years' experience with population health data analytics and/or value-based contracts.
