General Description:
The Population Health Analyst is responsible for assuring the validity and reliability of population health management (PHM) reports, including but not limited to daily huddle reports, HRSA UDS clinical quality metric dashboards, and patient outreach registries.
Location and Hours
The Primary Health Network Sharon, PA. (NOT a remote position)
Monday - Friday 8:30AM to 5:00PM
Duties:
-
- construct custom PHM reports by using all available software to clean and combine all necessary data from all reportable data sources
- validate and deliver each PHM report to meet its end use
- maintain the PHM data dictionary, report catalog, report criteria, report element mappings, and data quality monitor
- complete all other PHM report validity and reliability assurance tasks assigned
Job Requirements/Qualifications
· Bachelor’s degree in nursing, healthcare information technology, public health, or a field within the life or social sciences
- more than one year of full-time professional experience demonstrating an advanced capability to create custom Excel formulas and/or custom reports from SQL-based software in order to clean, summarize, and interpret information from large, complex datasets
- commitment to the purpose of community health centers (FQHC) and other safety-net health care provider organizations that provide care regardless of ability to pay
