Primary responsibility is to coordinate client care and resources for those who are mainly youth who have disabilities or with chronic illnesses by coordinating home care and acting as a liaison between medical providers, resource agencies and the client and families.

Job Duties:

Maintains caseloads
Conduct assessment and consultations with new clients.
Maintains client records client information accurately and confidentially.
Notify clients with basic information on caregiver, service start date and times caregiver will be providing care.
Participate in caregiver interviews to ensure appropriate selections are made to meet All-Ways Care Services, Inc. standards.
Orient caregiver with client information, directions, time and basic orientation of client's needs and diagnosis.
Input and maintain client information in computer system.
Answer and triage phones with professionalism, urgency, and calmness.
Processing time sheets and data care logs submitted by caregiver.

Perform supervisory / quality check visits with client to monitor service quality.

Qualifications:

College degree preferred

More than two (2) years of experience in the related field with care coordination

Computer proficiency with working knowledge in Microsoft Office, Excel, and ability to learn new software

Ability to communicate promptly and effectively with caregivers, case managers, clients care team, clients and family members regarding scheduling issues

Ability to deal with sensitive information and maintain confidentiality

Attention to detail and strong organizational skills

Flexible, able to adapt rapidly to changing priorities and tasks; willing to learn new tasks

Ability to work in a team environment

Experience in home care staffing

Ability to multi-task Willingness to provide client care if need arises

Excellent organizational, customer service, telephone, interpersonal, and oral and written communication skills needed.

Job Type: Full-time

Pay: From $15.00 per hour
