Rennes provides care to residents and patients who are with us for short-term rehabilitation, outpatient services, assisted living and long-term care. As a Physical Therapist Assistant you work under the direction of a Physical Therapist & Director of Rehab. You are responsible for implementing the PT plan of care, carrying out treatment, updating goals, planning the discharge and collaborating with the PT and other health professionals to ensure resident and patient needs are met.

We are seeking a Physical Therapist Assistant who:

• Plans, organizes, and implements a physical therapy treatment plan
• Communicates effectively with the PT when there are changes with the resident or patient that require input by the PT
• Implements evidence and functional based practice
• Develops a safe and effective discharge plan for the patient
• Develops a rapport with residents, patients and family members to ensure a successful outcome
• Demonstrates cooperation, teamwork, and respect
• Demonstrates knowledge of concepts in the physical therapy process, quality assurance, goal setting and holistic approach to people
• Demonstrates ability to collaborate with the PT, OT, and ST staff as well as other members of the interdisciplinary team
• Demonstrates ability to establish and maintain effective working and public relations
• Demonstrates ability to treat resident and patient information as confidential
