Community Bridges is looking for a full-time Physical Therapist in Concord, NH!

We are a non-profit agency founded in 1982 with a mission to advance the integration, growth, and interdependence of people with developmental disabilities within their own communities. Building on the strengths of over 1,100 individuals in the greater Capital Area and their families, we work to identify barriers of access to find creative solutions to develop meaningful community engagement.

The Physical Therapist Participates in trans disciplinary assessments and the writing of assessment reports, providing expertise the areas of motor development assistive and adaptive equipment; participates in the development of the IFSP, provides individual treatment; service coordination, offers parent education and support. Also provides consultation to other staff and other agencies with whom we collaborate. Must have knowledge and comfort with providing home based services within a trans disciplinary model.

Qualifications:

Education: Bachelor’s Degree in Physical Therapy required. Master’s Degree preferred. Must have a valid New Hampshire State License in Physical Therapy or documented ability to obtain license within thirty days of hire.

Experience: A minimum of two years’ experience working in Pediatric (Ages 0-3) setting required; home based experience and experience participating on trans disciplinary team preferred.

Compensation and Benefits

The starting rate for this position is $45,000 per year. There is a $1000 sign-on bonus for this position - $500 will be paid at the time of hire and $500 will be paid after 6 months in the role. *Please note, sign-on bonuses are for new hires only. Re-hires and transfers are not eligible at this time.

Paid Time Off: For a full-time employee working 40 hours a week, 264.16 hours of Paid Time Off is received per year! PTO days will increase over the years based on longevity and there is no waiting period to start using your PTO!

Mileage Reimbursement: Employees are reimbursed for approved mileage driven on the clock.

Paid Training: As an employee, you are paid your usual rate of pay while attending training!

Specific Duties and Responsibilities:

• Serves families regardless of; disability race, religion, sex, ethnicity, illness, sociological or financial circumstances.
• Works flexible hours to meet family’s needs throughout service area.
• Works in variety of homes.
• Provides physical therapy in home/community environments for eligible children.
• Participates in trans disciplinary assessments.
• Writes multiple areas of assessment reports.
• Comply with State and Agency policies regarding confidentiality.
• Has strong knowledge base in typical and atypical infant and toddler development.
• Provides trans disciplinary services and physical therapy treatments.
• Attends and participates in group and individual supervision, administrative meetings, in services and other training events.
• Assists family members with management issues and coordination with other community agencies, including writing letters of medical necessity.
• Works closely with Family Support in meeting the families’ needs. Provides guidance, support, and information to families and helps to coordinate with community agencies.
• Maintains records and documents services as required.
• Maintains daily schedule according to assigned caseload.
• Provides parents with support, information, advocacy and assistance throughout services and transition process.
• Maintains up to date knowledge concerning Part C philosophy and regulations.
• Maintains New Hampshire PT certification as required by the state and other agencies.
• Keeps abreast of new developments in their field and attends relevant continuing education events.
• Other duties as assigned.

• Maintains training in and uses universal health precautions to protect self, staff, child, and family health.
• Represents Community Bridges services to the community and maintains a positive image in the community at large.
• Strong organizational skills.
• Strong written and verbal communication skills.
• Reports all and documents all cases of suspected abuse and neglect to supervisor and to the State of New Hampshire in accordance with state law and agency policy.

Work Environment and Physical Demands:

• Work Environment- must work flexible hours to meet family’s needs throughout service area. Works in a variety of home environments including those with having pets, those which are not handicapped accessible and those having tobacco smoke. Must effectively manage the stress of juggling multiple tasks and meeting deadlines.
• Physical Demands— standing, walking, sitting, and lifting is required. Must be able to physically lift children and move children’s equipment up to 40 lbs.

Travel:

Will be expected to drive to and from families’ homes 15-20 hours a week. Must have a valid driver’s license, proof of good driving record, proof of adequate auto liability insurance and reliable transportation.
