Job Description: Personal Support Specialist are used in conjunction with Supported Living Coach. Personal Support Specialists will follow the Implementation Plan (IP) of the individual, provide companionship, and personal care and may assist with daily living activities and other duties necessary to maintain the quality of life for each person in supported living.

Responsibilities and duties of this position include:

• Adheres to and is knowledgeable of Challenge Enterprises of North Florida Policy and Procedures.
• Participates in Challenge Enterprises of North Florida New Hire orientation, attends mandatory training and staff meetings.
• Follows established procedures for training and evaluating individual in his or her own home, in the community, and in the workplace.
• Responsible for assisting individuals with grocery shopping and meal preparation.
• Responsible for assisting and training in activities of daily living such as in the areas of eating, bathing, dressing, and personal hygiene.
• Responsible for improving the quality of life for assigned individual.
• Responsible to assist with heavy household chores to make the home safer including washing floors, windows, and walls, tacking down loose rugs, or moving heavy items/furniture.
• Strategically facilitate activities and care of the individual.
• Follows through with completion of directions.
• Follow the established Challenge Enterprises Vehicle Policy.
• Responsible for completing daily case notes in approved data collection system, medication log, and safety and health procedures.
• Responsible to train individuals to use public transportation
• Will be responsible for accurately recording time and attendance via approved time keeping system and promptly communicating any issues to Supported living Coach/Personal Supports Supervisor or Designee.
• Communicate between Supported living Coach/Personal Supports Supervisor or Designee the needs of the individuals.
• Communicate between Supported Living Coach/Personal Supports Supervisor or Designee, Waiver Support Coordinators, and family members regarding the needs of the individuals.
• Submit ideas and solutions to issues in the home, based on observation to Supported Living Coach/Personal Supports Supervisor regarding appointments with individuals in the home.
• Other duties as assigned.

Required Qualifications:

• Meet state requirements for background check.
• Current Florida Driver's License and 3 year clean driving record.
• Flexibility with work schedule - ability follow work schedule.
• Ability to lift 40 lbs.
• Ability to be physically active for extended periods of time.
• Creative and flexible.
• Excellent basic math and writing skills.
• Excellent communication and organizational skills.
• CPR and First Aid certified preferred

Education:

High School diploma or GED. At least one year of experience working in a medical, psychiatric, nursing or child-care setting or working with individuals who have a developmental disability. College or vocational/technical training that equals at least 30 semester hours, 45-quarter hours of 720 classroom hours may substitute for the required experience.

Physical Demands:

Physically able to climb stairs, stand, stoop, and bend for a least three hours per day.
