Personal Care Aide (PCA) - Day Center

The P*rogram of All-Inclusive Care for the Elderly (PACE)* is a unique health care model designed to help seniors (participants) stay safely in their home for as long as possible. PACE coordinates and provides or facilitates all medical care for its participants. Services are delivered across various settings, including on site at the PACE center, in the community, and in participant homes.

Responsibilities Will Include:

Performs patient care activities and other tasks as assigned to assist the primary care, therapy, and adult day health staff. Works under the direct and indirect supervision of the Clinic Manager or designee but is independent in the application of personal care aide knowledge and skills. Serves as an active Day Center team member to promote a positive experience for PACE participants who benefit from opportunities to engage in therapeutic and social community opportunities outside of their homes.

• Communicates effectively with participants, families, staff, and the staff of other agencies.
• Assists in facilitating day center activities with guidance from the Recreational Therapist.
• Works effectively in a team setting.
• Provides basic ADL/IADL care to day center participants including but not limited to toileting, bathing, meal preparation/feeding, vital monitoring.
• Documents treatments/procedures, observations, personal care, and participant activities on appropriate forms and in the medical record as appropriate.
• Reporting changes in the participant’s condition to the pertinent Interdisciplinary Team member.
• Demonstrates active participation in QI processes.
• Participates in the process of teaching and learning with others.
• Abides by all established PACE Central Michigan policies, rules, and regulations.

Education and/ or experience and Licensure:

• Must possess, as a minimum, a high school diploma or equivalent.
• Certified in Michigan as a Certified Nursing Assistant.
• Must be CPR certified.
• Valid Michigan Driver’s License
