PEDIATRIC REGISTERED NURSE AS HOME CARE CASE MANAGER

Florida licensed Registered Nurse with strong pediatric background in the care of medically complex infants, children or young adults needed for home care clinical management position with ChildrenFirst Health Care.

Requires 3-5 years recent experience from intensive care units including peds, neo, or neuro preferred. Family practice or peds clinic background considered.

Responsibilities include: clinical management of assigned case load through collaboration with physicians, hospital discharge professionals, parents, bedside clinical team members, and fellow case managers. Position rounds to patients' homes, generates required clinical documentation through EMR system, participates in parent and nurse teaching/training, rotates on-call, attends MD appoints as needed. References must demonstrate clinical competence, dependability, professional appearance and attitude, able to team build, detailed, thorough, solid computer skills, able to work with diverse patient and nurse populations.
