Summary

We are looking for a Patient Navigator that will sit out of our Downtown Chicago clinic. This will be a full time, full-benefits, 40 hours a week position. The hours will be Monday-Friday, 8:00am-5:00pm.

As a Patient Navigator you will work closely with the patients, nursing, medical assistants, and billing team, Responsibilities for this position may include but are not limited to coordinating with nurses/medical assistants to provide superior care, answering basic patient questions, obtaining authorization, and documenting all interactions with patients.

Responsibilities

  • Establishes close relationships with and serves as primary point of contact for patients
  • Support the IVF team members with all aspects of patient treatment coordination, such as patient communication and education, scheduling, documentation, and record-keeping
  • Teach patients about their care options and make specific recommendations based on their goals
  • Maintain documentation of all patient encounters and complete reporting requirements according to organization standards
  • Track patient information, schedules, files, and forms in a confidential manner
  • Explain test results, diagnoses, and other medical outcomes to the patients affected by them
  • Assist and interact with patients in person, by telephone, and via email/text
  • Collaborate with team members to deliver a high-quality patient experience
  • Utilize nAble EMR system to ensure patient records are up-to-date and secure
  • Provide support to the team with other administrative tasks as necessary
