Cleerly is creating a new standard of care for heart disease through value-based, AI-driven precision diagnostic solutions with the goal of helping prevent heart attacks. Our technology goes beyond traditional measures of heart disease by enabling comprehensive quantification and characterization of atherosclerosis, or plaque buildup, in each of the heart arteries. Cleerly's solutions are supported by more than a decade of performing some of the world's largest clinical trials to identify important findings beyond symptoms that increase a person's risk of heart attacks.

About the Opportunity

As a Patient Care Coordinator, you will be joining the Strategy team as we navigate how Cleerly can expand to better engage with and serve patients. You will own patient care coordination, including outreach, patient engagement, data entry, patient scheduling, prior authorizations, and collection of payment.

  • Communicate via email and phone to engage with patients
  • Work to improve the patient experience and increase pull through
  • Enter data and track patient journey, sort information to prepare data for reporting
  • Foster continuous improvement through planning and workflow development
  • Schedule CCTA exams at integrated and non-integrated imaging centers according to patient needs and preferences

Salary: $55,000 - $80,000*

  • Total Target Compensation (TTC): Total Cash Compensation (including base pay, variable pay, commission, bonuses, etc.)

Responsibilities

  • Liaison between patients, imaging centers and providers
  • Communicate and engage with patients who may need prior auths in addition to scheduling CCTA appointments with imaging centers nationwide
  • Log data in real time to provide weekly and/or monthly reports
  • Create a positive patient experience
  • Key point of contact for care coordination, answer questions and concerns

Requirements

  • 5+ years of relevant experience
  • Worked in a client-facing role in a physician's office or clinical setting
  • Team player with excellent communication and presentation skills
  • Organized and can manage multiple patient journeys at a time
  • Experience with prior authorizations
  • Self-motivated and willing to jump into tasks as they arise
  • Quick learner and problem solver that works well under pressure
  • Flexibility and agility to work cross-functionally in a fast-paced environment
