Nexus Family Healing is seeking a Part Time Weekend Nurse (RN) to join the new Southeast Regional Crisis Center (SERCC) based in Rochester, Minnesota.
This is an exciting opportunity for an individual who is passionate about mission-driven work while aligning communities and regional partners to provide the right kind of care for individuals experiencing mental health crises across southeast Minnesota.

Position Summary for the Nurse (RN):
Promote health and well-being to those served, by performing client-centered health care services, in accordance with the Southeast Regional Crisis Center’s (SERCC) policies, procedures, and protocols.
We are looking for either a part time or full time nurse and this position offers flexible scheduling and a paid commute program for eligible employees!

Responsibilities:

  • Coordinate the delivery of medical care to residents.
  • Collaborate with other interdisciplinary teams regarding medical aspects of client admissions.
  • Communicate medical concerns and coordinate necessary follow-up appointments, and/or referrals with interdisciplinary teams both internally and externally.
  • Collect and review clients’ medical data and ensure that medical records are current and accurate.
  • Assess nursing implication of all data pertaining to the resident and determine the need to communicate with the physician.
  • Monitor acute and chronic medical conditions of residents; provide treatment or consultation, as needed.
  • In emergency situations, gather relevant information and assess client status. Provide appropriate care and/or treatment, direct others as necessary, and collaborate referral to a higher-level of care if appropriate.
  • Collaborate with members of the treatment team to develop Individual Treatment Plans
  • Serve as a resource to staff and other interdisciplinary teams regarding client medical concerns.
  • Educate clients on medication side effects, adverse effects, and drug interactions.
  • Monitor clients daily for side effects, adverse effects, and/or drug interactions.
  • Ensure client prescriptions are filled and the appropriate amount is maintained through each client’s stay within the residential milieu.
  • Implement, monitor, and coordinate the administration of medications to clients.
  • Monitor medication logbooks for completion and accuracy.
  • Verify, count, and administer medications.
  • Attend and participate in staff meetings and training sessions as required. Understand and support the Nexus philosophy and treatment modalities.
  • Complete all required trainings that are department/role specific & facility-wide requirements.
  • Maintain and stay up to date with trainings and certifications (i.e., online trainings, internal and external in-person trainings, CPR certification, nursing license, first aid and safety, medication administration, and infection control etc.
  • Participate in internal training opportunities with community partners.
  • Ensure client information remains confidential and comply with Nexus policies involving client privacy, informed consent, and confidentiality standards for clients and their families.
  • Attend and participate in required staff meetings.

Education, Credentials and Experience:

  • Graduate of an accredited school of nursing.
  • Valid Registered Nurse license.
  • Minimum of 1 year related experience.
  • First Aid/CPR/Infection Control certification.
  • CPI/SCM certification may be required, depending on agency-specific licensing requirements.
  • Valid driver’s license and a driving record that satisfies regulating agency and corporate policy.

Physical Demands:

  • Must be able to lift up to 50 pounds unassisted and up to 100 pounds with assistance.
  • Must be able to stand and walk for extended periods of time (2-4 hours).
  • Must be able to twist, bend and stretch in a manner conducive to daily activities within a residential treatment facility.
