Clifford Beers is a mental health clinic serving children and families. Our mission is to provide integrated services addressing mental, physical, and social determinants in order to improve health, resiliency and quality of life for children, families and communities.

*
PURPOSE: *
The individual will engage families who have received a Family Assessment Response from DCF. The Parent Navigator will work as part of the team which includes the family, Community Support Worker and other involved supports to facilitate the engagement of the family. The Parent Navigator will connect the family to concrete, traditional and non-traditional supports within the community.

KEY RESPONSIBILITIES: *

  • Working collaboratively with the Community Support Worker, family and involved supports
  • Utilizing the wraparound family team model
  • Traveling within the Greater New Haven service area to provide services
  • Partnering with the family and other supports, to develop a strengths-based, family-driven plan of care
  • Participating in child and family team meetings
  • Advocating, empowering and mentoring families to take the lead in their own plan of care
  • Documenting all contacts with families/ supports
  • Bilingual Preferred
  • Other duties as assigned by Supervision

  • Bilingual Spanish a plus

EDUCATION and EXPERIENCE: *

  • High School Diploma
  • Minimum of 3 years’ experience with families in need

REQUIREMENTS: *

  • Valid driver’s license and reliable transportation

TRAVEL: *

  • This role may require travel throughout the Greater New Haven area.

Physical Abilities: *

  • 25% of the time is spent making repetitive motions.
  • 35% of the time spent sitting.
  • 20% walking, bending, standing, stooping, crouching or kneeling.
  • 25% of the time is spent using hands to type, write, or use controls
