JOB SUMMARY:

• is individual possesses expertise that qualifies him or her to perform all nutrition-related duties at the local agency, including providing and overseeing high risk care participants.
• Conduct a complete nutrition assessment, determine nutrition risk and enroll participants.
• Conduct a breastfeeding assessment, and provide participant centered counseling that helps a mother initiate breastfeeding, establish a milk supply, exclusively breastfeed and continue breastfeeding.
• Address common breastfeeding concerns and issue breast pumps.
• Refer high-risk participants to other health-related and social services, as necessary
• Prescribe, tailor, authorize and reissue food package.
• Provide and document participant-centered nutrition education/counseling (individual/family sessions and facilitated group discussions)for participants.
• Ensure the efficient flow of the assessment and education processes.
• Track high-risk participants’ progress in improving their health and document for outcomes. Provide leadership in planning, conducting and evaluating participant-centered nutrition services.
• Provide leadership in developing, conducting and evaluating nutrition training for LA staff in nutrition education topics.
• Provide leadership in the planning, conducting, and evaluation of Participant-Centered Nutrition Education services.
• Perform other related duties as assigned by the Site Supervisor and Program Director.
• Attend and participate in monthly department, All Staff and other required meetings.
• Be familiar with and adhere to all clinical, administrative, and general policies and procedures as outlined in the Cornerstone Family HealthCare Policy and Procedure Manual and Employee Handbook.
• Maintain confidentiality of all aspects of Cornerstone Family HealthCare including, but not limited to, patient confidentiality, Financials, and employee relations.
• Perform other duties as assigned.
• Bilingual Candidate a Plus

• Bachelor’s Degree in Nutritional Sciences, community nutrition, clinical dietetics, foods and nutrition from a regionally accredited college or University, OR one recognized by New York State Education Department Registered Dietician (RD) and/ or RD-eligible preferred
