Full-Time Family Practice Nurse Practitioner in Northeastern Colorado!

We are a critical Access hospital with 5 Acute Care Beds, 20 Extended Care beds (SNF), and 2 Clinics.
Low Volume/low acuity, Level IV Trauma, mostly stabilize and transport. 500 ER visits/yearly
Full-Time = 4 days a week (clinic, hospital, and extended care) + 8 shifts of call per month.
Town of ~1,000 people. Great town and culture; good schools, no crime, strong sense of community, good people, 2 hours from Denver, very easy drive.
Currently 2 physicians and 1 Nurse Practitioner on staff.
Annual Salary $125K Additional salary potential of $25K for extra call shifts.
The Nurse Practitioner will plan, direct, and evaluate resident and employee health care in ambulatory health services; examine and treat chronic and acute episodic illnesses, minor injuries, accidents, and other injuries/illnesses; performs selected laboratory testing; offer referrals to physicians, write prescriptions, and perform office procedures within the scope of expertise, protocols, and available equipment. The Nurse Practitioner will also work with the medical director in the care and treatment of residents and staff.
Duties/Responsibilities:

  • Meets with new residents to orient them about services, to identify needs, and to suggest local physicians and services.
  • Sees ambulatory patients in the health services office for laboratory testing, examination, diagnosis, and treatment of chronic and acute episodic illnesses and minor injuries. Refers to physician as needed. Writes prescriptions and performs office procedures within the scope of expertise, protocols, and available equipment. Makes house calls when necessary. Schedules follow-up visits in office or unit as needed.
  • Performs physical examinations (pap tests, etc.)
  • Performs staff employment physicals, TB testing, etc. Examines staff after on-the-job accidents or injuries.
  • Acts as a resource person for decisions, planning, and policies affecting the health of residents and staff.
  • Maintains outpatient records. Records statistics and conducts research as deemed necessary by institution. Compiles reports concerning ambulatory care at facility.
  • Collaborates with staff to identify residents' needs and to help develop plan to meet these needs.
  • Follows residents in health center as needed for continuity of care.
