Community Options, Inc. is a national non-profit agency providing services to individuals with disabilities in 11 states.

We are seeking a Medical Coordinator in Pittsburgh, PA! The Medical Coordinator will be under direction of the RN. This person will plan, track, monitor and ensure that all clients’ needs are being met within appropriate time frames. Primary responsibilities include; the scheduling of all annual, attend, schedule, and follow-up on emergency and non-emergency medical appointments. These include communicating with medical professional staff, program specialists, supervisors and program directors. The Medical Coordinator delivers care to individuals with disabilities in a community setting utilizing the nursing process of assessment, planning, intervention, implantation and evaluation. The Medical Coordinator effectively interacts with the patient, significant others, and other health team members while maintaining standards of professional nursing.

Available Shift: Monday through Friday

Pay rate is: $20.00 an hour

Responsibilities

  • Participates in the admission of new residents to assigned homes.
  • Develops plans to guarantee that the medical and healthcare needs of the residents are met.
  • Reviews lab work and coordinates annual physical exams and medical care.
  • Provides special instructions to resident and home staff related to medical diagnosis, allergies, diet, etc., and is available for ongoing consultation to staff regarding medical problems and healthcare.
  • Clarifies medication orders and contacts physician to review individual’s medication plans.
  • Participates in quarterly and annual Individual Service Plan (ISP) meetings for assigned participants.
  • Creates and distributes Individual Service Plan Medical/Health Reports detailing participants’ general health, health care goals and physician recommendations for each participant.
  • Provides documentation regarding individual’s health status for various support people including family, guardian, staff, employment and other professional staff as necessary.
  • Monitors participant’s use of medication through Medication Administration logs.
  • Makes referrals for needed medical services and ensure that follow-up care is pursued.
  • Provides pre-service training for all new support staff on consumer specific medical needs.
  • Provides quarterly in-service training for support staff.
  • Assures that health services supplies are available, and places supply orders when necessary.
  • Advises supervisor of all concerns and/or issues related to the provision of nursing services.
  • Follows strict OSHA procedures regarding blood-born pathogens.
  • Participates in Circle(s) of Support for personal future planning.
  • Attends meetings as needed, based on consumer needs.
  • Attends conferences, seminars and training as required by licensure or supervisor.
  • Teaches and reinforces vital signs and medication management to home staff.
  • Cooperates with internal and external investigations.
  • Additional tasks and responsibilities may be assigned.

Minimum Requirements

  • Bachelor’s Degree or equivalent experience in nursing field.
  • Minimum of two years of experience as a registered nurse.
  • Valid driver’s license with satisfactory driving record.
  • Valid state nursing license.
  • First Aid, CPR, OSHA and Medication Certification course within first 90 days of employment, and any required residential or state training.
  • Crisis management skills and consistent demonstration of ability to respond to changing situations in a flexible manner to meet current needs.
  • Ability to effectively and professionally communicate with program participants, family, direct care professionals, other agencies and members of the community.
  • Ability to calculate figures related to medical diagnosis.
  • Ability to establish a comfortable and supportive relationship with a person receiving support.
  • Excellent oral, written and interpersonal communication skills.
  • Ability to demonstrate strong leadership skills.
  • Awareness of the needs of the developmentally disabled.

Working Conditions

  • Work in varied residential environments based on assigned caseloads
  • Schedule may change due to business needs and may include evening and weekend hours
  • On call, via cell phone, 24 hours a day for medical emergencies
  • Frequent lifting, stretching, and other physical exertion may be required
  • May assist with wheelchair transfer of non-ambulatory individuals
  • May be exposed to various medical conditions and communicable diseases
