We are currently seeking a skilled and dedicated MDS Coordinator to join our elite team and provide outstanding and innovative skilled healthcare in the comfortable and friendly environment of two facilities.

As the MDS Coordinator, you will be responsible for conducting physical assessments of residents on admission, coordinating resident assessments, and scheduling MDS reviews and updates. You will work closely with staff from multiple departments, as well as families, to coordinate care processes and develop care plans that are tailored to meet the individual needs of each resident.

Job Type: Full-time

Pay: $24.00 - $34.00 per hour
