Lorien Health Services has an opening for a RN MDS Coordinator for our Mays Chapel Community which is located in the beautiful Mays Chapel/Lutherville area, conveniently found off of 83N/S and York Rd.

Responsibilities and Duties

The Minimum Data Set (MDS) 3.0/PPS Coordinator directs the completion of the resident assessment and schedules the care planning process in accordance with Federal and State guidelines for a group of residents of the facility’s adult/geriatric population.

Qualifications and Skills

• Current Maryland RN License with strong MDS 3.0 training and experience
• Progressive nursing experience preferably in Long Term Care/Skilled Nursing Facility
• Competency with computer software
• RAC-CT "Resident Assessment Coordinator certified" required
• TEAM oriented
