Lakeshore Manor Nursing & Rehab in Slidell, Louisiana is a new facility that is looking to hire a dynamic Full-time Social Services Director to work in our medical care facility.

Responsibilities include:
- Work closely with the medical team to develop patient care plans
- Responsible for making clinical decision, exercising judgement to ensure best possible care is provided within available resources
- Provide counseling services, with particular regard to individual and family issues and decisions that impact the health of the patient and family
- Assist in discharge planning which includes ensuring seamless transition to community resources
- Act as an advocate for patients and their families in matters affecting the patients health
Education Requirements:
- Must be a Licensed Social Worker
- Minimum 2-year experience, preferred
- Previous related patient care experience, preferred
- Must be knowledgeable in community and the resources available
