Job description
We're seeking a PT weekend LPN-Memory Care Clinical Coordinator to manage our Memory Support and Assisted Living communities. This is an hourly position from 8am-6pm (weekends).
- Comprehensive knowledge of practices and procedures of long-term care; practices and procedures for clinical documentation
- Provides clinical leadership to household team and councils to develop policies and parameters for meeting the needs of residents.
- Quality Improvement: Maintains continuing quality assessment, improvement, and evaluation of service delivery.
- Resident Care: Plans
- Conducts daily clinical rounds compliance.
- Care Planning
- MDS completions
- Minimum of two (2) years of supervisory experience preferred.
- Minimum of three (3) years’ experience working with seniors in a skilled healthcare facility, preferably in memory care.
- Care planning experience preferred.
- Must be a graduate of an accredited and certified school of Nursing.
- Bachelor’s degree or Master’s degree from a recognized Nursing program preferred.
- Must be a licensed LPN in good standing in the State of Kansas or compact state.
- Must obtain AED certification within 90 days of hire.
