QUALIFICATIONS: High school diploma, successful completion of an accredited Practical Nursing Program, Board exam, and licensure to practice in NJ. Minimum one year of LPN experience; clinic or LTC setting experience preferred. At least 1 year working with frail and elderly population. Computer experience required. State-licensed practical nurse, Current CPR Certification, Valid driver's license.

Position Description

A nonexempt position responsible for nursing care under the direct supervision of the RN Clinic Manager. Position involves medication administration, wound care; providing nursing care as ordered in day center, clinic and/or homecare settings.

As a multisite provider of PACE community-based services for seniors in Cumberland, Gloucester, and Salem Counties, an Inspira LIFE employee may work at either or both the Vineland or Williamstown LIFE Program as census changes or operational needs occur to meet the needs of our participants.

Position Responsibilities
    • Dispensing and documentation of medications as per PCP orders.
    • Perform wound care, as ordered by PCP.
    • Screen participants for evaluation by PCP.
    • Assist with prioritizing appointments to see PCP.
    • Obtains vital signs, weights, and identifies reason for visit prior to exam by PCP.
    • Obtains EKGs as ordered.
    • Obtains lab work as ordered.
    • Educating participant on follow-up visits and specific orders.
    • Home Visits to ensure compliance with medication, follow-up with wound care orders.
    • Provides CPR if needed.
    • Medication reconciliation.
    • Assist with change of oxygen tanks when needed.
    • Documents all care in EMR timely and accurately.
    • Assists with Scheduling of necessary appointments of participants for clinic.
    • Performs physician order reconciliation post ED/Hospitalizations, Specialists' visits, Pain Mgmt. Clinic, etc.
    • Consults each day with the Clinical staff on managing the participant flow for the day. Assists in prioritizing the scheduling of participants.
    • Greets participants that enter the Clinic with a smile and pleasant voice and maintains eye contact.
    • Maintains the confidentiality of all procedures, results and information about participants, clients, or families.
    • Performs other duties as requested by immediate supervisor.
    • Competent in documenting in EHR as required.
    • Immunizations to participants and accurate documentation into the EHR.
