LPN Charge Nurse with New Wage Scale**

This LPN will supervise the day-to-day nursing activities in accordance with the Nurse Practice Act, current federal, state and local standards, to ensure the highest quality of care is maintained at all times. LPN – Available Schedules:

· Day Shift: (7:00 am-3:30pm)

· Evening Shift: (3:00 pm- 11:30pm)

· Night Shift: (11:00 pm- 07:30 am)

· Weekend Warrior: Every Saturday and Sunday; 16 hour shifts (7a-11p OR 3p- 7a); may have 1 weekend off within 6-week schedule

LPN – Responsibilities:

· Direct the day-to-day functions of the Nursing Assistants in accordance with current rules, regulations, and guidelines that govern the long-term care center

· Assist in planning the nursing services portion of the residents’ discharge plans as required

· Complete accident/incident reports as necessary

· Chart nurses’ notes in an informative and descriptive manner that reflects the care provided to the residents, as well as the residents’ responses to the care

· Prepare and administer medications as ordered by the physician in accordance with federal, state, and local regulations and as required by center policy

· Order prescribed medications, supplies, and equipment as necessary and in accordance with established policies

· Ensure that narcotics are handled in accordance with federal, state, and local regulations and as required by center policy and that narcotic records are accurate for your shift

· Make daily rounds of your unit/shift to ensure that nursing service personnel are performing their work assignments in accordance with acceptable nursing standards

· Notify the residents’ attending physicians and next-of-kin when there is a change in the residents’ conditions or if the residents are involved in an incident or accident

· Attend and participate in all center in-service programs as required

LPN – Qualifications:

· MUST hold a current and valid LPN license in the state employed

· Long term care/post-acute care setting preferred

· Completion of post‐secondary education

· Capable of maintaining regular attendance (i.e. weekend shifts and overtime) as required
