Our creative team of social service providers is looking for a CASE MANAGER to offer housing support to a small caseload of Foundational Community Supports eligible clients. What you’ll do as our FCS Case Manager:

  • Provide “supportive housing support” services including, but not limited to: housing assessments, identifying housing resources, support obtaining a lease, independent living skills development, landlord relations and crisis management.
  • Develop expertise in the supportive housing arena by working with local housing authorities, providers, and partners and additional resources regarding affordable housing availability, subsidies, voucher capacity, and supportive wrap-around services.
  • Maintain accurate records on housing search process for verifiable billing. Document and communicate with clients, agency and community partners on housing search progress.
  • Maintain individual client case load and manage multiple deadlines, appointments and reporting requirements.
  • Participate in community learning opportunities about affordable housing programs and supportive services in the region.
  • Actively collaborate with other programs within FLC agency as well as community partners and housing managers to provide the best person-centered support possible for FCS clients.
  • Conduct outreach throughout King County, including transportation necessary to provide supportive housing services (including direct transportation of clients as needed via car, bus, or ride-share service) and respond to individual case needs.
  • Provide appropriate resources to assist in independent living, community integration, including social services to obtain and meet medical care and holistic person-centered needs.
  • Develop a person-centered plan and assessment to meet the needs of community integration to include short and long-term goals as well as goal-driven action plans to address client needs and concerns.
  • Provide support in landlord-tenant communications, including independent living skills, financial counseling, and management.

What you will need to succeed!

  • Knowledge of principles, methods, and procedures of providing outreach-based community support services to support client ability to maintain independent living situations.
  • Two (2) years of human service experience in the coordination of Supportive Housing or in the coordination of independent living services in a social service setting.
  • Case management experience working with housing or other community services.
  • Either a valid Washington State Driver’s License and acceptable driving record or access to public transit.
