About the Opportunity/Work

RESPONSIBILITIES/ESSENTIAL FUNCTIONSThe Hospital Liaison/Crisis Case Manager is responsible for following the acute care and crisis contact protocols for all Specialized Services for Youth (SSY) individuals admitted to an acute hospital or PHP program or who have had a crisis contact with New Oakland. The Hospital Liaison/Crisis Case Manager completes all paperwork, phone calls and sessions within required timeframes and coordinates all information with the assigned clinician. The Hospital Liaison/Crisis Case Manager provides direct crisis intervention services as needed. The Hospital Liaison/Crisis Case Manager assesses for suicide intent and implements prevention strategies with the individual and family as needed, in coordination with the assigned clinician. The Hospital Liaison/Crisis Case Manager also serves as back-up to the Medical Case Manager as needed. The Hospital Liaison/Crisis Case Manager position acts cooperatively as a team member with other SSY staff and reports to the Pontiac Outpatient Supervisor. The following represents a summary of the major duties and responsibilities performed by incumbents in this position. Incumbents may be requested to perform related tasks other than those specifically presented.
SPECIFIC DUTIES/JOB EXPECTATIONS

  • Upon notification of an SSY individual being admitted to an acute psychiatric facility or Partial program, arranges a face-to-face visit at the hospital with three calendar days of admission.
  • Upon notification of an SSY individual receiving a crisis contact through New Oakland, arranges a face-to-face visit in the home/office/community within three calendar days of the crisis contact.
  • Provides direct crisis intervention services to the individual/family as needed.
  • Assesses for suicide risk and provides early intervention/prevention strategies to the individual and family
  • Coordinates regularly with assigned clinician, updating the clinician on any contacts and obtaining information.
  • Completes all necessary paperwork including IRs and PN/CNs in ODIN as appropriate within required timeframes.
  • Completes hospitalization tracking spreadsheet within required timeframe.
  • Acts as back up to the Medical Case Manager in absences and performs all necessary functions to ensure quality care, including scheduling psychiatric appointments, facilitating tele-health sessions, communicating with psychiatrist and/or assigned clinician related to medications or behaviors, takes and records vital signs, coordinate with Primary Care Physicians to support Integrated Healthcare Efforts.
  • Utilizes ODIN and any other technology needed to complete responsibilities.
  • This position will assume responsibility in partnership with the "team" in identifying and eliminating barriers to service.
  • Performs other duties as assigned by the supervisor.

Does This Describe YOU?*

  • Bachelor’s Degree in Human Services area or related field.
  • Minimum one-year experience working in a setting that serves children/ adolescents with behavioral/emotional issues and other special needs.
  • Selected candidates for this position must possess a sensitivity to the diversity of the Agency’s service population including (but not limited to) differences of culture, race, religion, gender, sexual orientation, and socioeconomic characteristics.
  • Possess a valid driver license and insurance as specified by OFS.
  • Demonstrated skill in office procedures, data entry, use of computers and human services.
  • Well-developed verbal and written communication skills.
  • Demonstrated knowledge of the principles of trauma informed care.
