We are seeking a qualified Full-Time Registered Nurse for our Hospice office. This office is located in Newport News, VA and provides service to the cities in the Tidewater area that includes Newport News, Hampton, Yorktown, Poquoson, Williamsburg, Gloucester, Norfolk, VA Beach, Portsmouth, Chesapeake, Suffolk and Smithfield.

QUALIFICATIONS

Licensed Registered Nurse with current valid license to practice in Virginia
1 year Hospice experience required with OASIS experience preferred
Knowledgeable of State and Federal regulations
Strong computer skills required; familiarity with Allscripts preferred
Excellent observation, verbal and written communication skills, problem solving skills, basic math skills
JOB SUMMARY

The Registered Nurse plans, organizes and directs hospice care services and is experienced in nursing, with emphasis on community health education/experience. The professional nurse builds from resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities.

ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES

Completes and initial and comprehensive assessment of patient and family to determine hospice needs.
Regularly re-evaluates patient and family/caregiver needs
Develops a plan of care that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions in concert with the Interdisciplinary group
Participates in on-call duties as defined by on-call policy
Attends death visits.
Performs chart reviews, prepare interdisciplinary meeting notes
