For this particular role, we are looking for looking for an experienced Hospice RN who is ready to take on the challenge of building, leading, and supporting their team of dedicated Hospice RNCM's, and help take Namaste to new heights!

I. JOB SUMMARY

The Hospice Clinical Manager is the leader of the Patient Care Team. In this role you will supervise, evaluate, and coordinate various members of the interdisciplinary team; assure continuity of care from admission to discharge or transfer to bereavement; Administrative duties as in hiring, corrective action, and employee reviews. The patient care manager will go out in the field with the nurses, training and coaching their team. The Patient Care Manager is responsible for ensuring that care and services are delivered appropriately and for the supervision of clinical personnel

II. DUTIES & RESPONSIBILITIES

1. Lead/support a team of Interdisciplinary professionals who are taking care of patients at the bedside every day. Helping them to meet the needs of the hospice patient and family in a manner consistent with the guidelines and policies of Namaste Home Health and Hospice

2. Coach, influence and mentor your team to strive for excellence in Life-changing service.

3. You will Provide supervision and counseling to Coordinators and Clinical Supervisors assisting them in continually improving all aspects of hospice care services, provided through organization personnel

4. You will be the hub for communication regarding patient care and will promote a positive working relationship with the family, physician, and others involved in their care

5. Supporting the intake team by clinically reviewing referral/inquires in a timely manner.

6. Performs chart audits and triple checks. while supervising the maintenance of medical records, assuring completeness and compliance with regulations, certification standards, legal and ethical standards

7. Participating in the development and implementation of orientation and in-service training of hospice programs for all levels of nursing staff employed by the hospice program

8. Appraise clinical staff performance during their probationary period and annually

9. Formally meets with reporting staff for 1:1 meeting quarterly. Informally weekly.

10. Provides the teaching and counseling necessary to enhance the delivery of services from nursing and health aide staff

11. Reviews and evaluates each case through a variety of means such as home visits, conferences, record review and the services provided by clinicians; discusses and verifies impressions, instructs and guides clinicians to promote more effective performance and delivery of quality home care services; and is available at all times during operating hours to assist clinicians as appropriate.

12. Assists clinicians in establishing immediate and long-term therapeutic goals, in setting priorities, and in developing plan of care.

13. Participates actively in quality assessment performance improvement program.

14. Leads regular interdisciplinary team meetings for the development, review, and revision of patient/family Plan of Care, including exchange of information, review of problems or unique issues from caseloads, and sharing of professional support

15. Support team members by working both in the field and in the office.

16. Attends and participates in: Namaste’s Leadership Development Program, Clinical Accountability Meetings, IDG, and other meetings as necessary.

17. Participates in the Administrator on Call Rotation.

18. Responsible for completing assigned clinical workflow tasks daily.

Job Type: Full-time

Pay: $90,000.00 - $110,000.00 per year
