Home Health: Occupational Therapist

ESSENTIAL DUTIES AND RESPONSIBILITIES: This position is directly supervised by the Program Administrator, Therapy Coordinator, and/or Director of Nursing within community Health Network, LTD.

Consult with HHA team to select activity programs and coordinate occupational therapy with COTA

Provide plan of care for treatment for the patient

Evaluate and modify treatment plan based on individual progress (re-certifications)

Comprehensive evaluation of home and job environments and recommendations on necessary adaptation

Provide direction and plan of care to the COTA
-
Provide supervisory visits to the assigned COTA

Input documentation into the organizations software data base for billing

Attend scheduled and/or make arrangements with the Administrator to attend required organizational and accreditation required training.

Job Location: Wayne County

High school diploma, GED, or Equivalent

Copy of Occupational Therapist license
