Managing Care of Residents:

The Health Services Coordinato is responsible for ensuring that all residents care needs are being delivered in a professional manner with dignity. The Health Services Coordinator communicates with residents, family and staff on an as needed basis to meet resident needs. The Health Services Coordinator organizes and completes Care Conferences every quarter with residents and family members to discuss resident needs and appropriate service delivery.

Managing all Direct Care Staff in the Clinical Department:

The Resident Care Coordinator is responsible for screening, hiring, training, and supervising all caregivers and medication technicians at the community. This includes managing poor performance, coaching to improve skills, managing activities to correspond to care plans and appropriate documentation and compliance with all human resource guidelines. Assures that all staff maintains appropriate annual training to keep up with state requirements. The Health Services Coordinator is responsible for Scheduling Caregivers and Medication Technicians working iconjunction with the Resident Care Coordinators and be on call for staffing call outs every 3 weeks on a rotation.

Continuous Quality Improvement:

Health Services Coordinator is responsible for oversight and compliance with community CQI Program and takes steps to assure that services delivered are completed as designed.

Maintaining Compliance with all local, state and federal guidelines:

Health Services Coordinator is responsible for all local, state and federal rules and regulations as they apply to services delivered at community. This involves staff selection, training and oversight, resident care and documentation, and compliance with HIPAA, OHSA, Blood borne Pathogen Guidelines, and any other rule that applies to the work we do in regards to caregiving and Medication Technician services.

Safety Compliance:

The Health Services Coordinator is responsible for insuring that they and any employees reporting to them work in a safe manner and that unsafe actions are to be corrected. Compliance with Safety Committee Standards is required. Should workplace injuries occur, the Department Heads and Directors are to assist the Executive Director when required with the investigation, return to work, and management of the injured worker communication with care provider and community.

5. Team Effectiveness:

The Health Services Coordinator is responsible for fostering teamwork with co-workers, showing initiative in overcoming obstacles, taking actions which assists others beyond job responsibilities, and having a positive impact on co-workers, residents and visitors.

6. Perform other duties as assigned or needed.

Job Type: Full-time

Pay: $51,000.00 - $53,000.00 per year
