Responsibilities:
Responsible for the overall MDS completion and accuracy of Grace Lutheran Communities and its affiliates SNF facilities. They are expected to uphold the mission, philosophy and values of Grace Lutheran Communities to provide high quality, holistic care to our residents, staff, and community partners.

MDS Coordinators oversee and facilitate the completion of resident assessments at nursing facilities. Throughout the assessment and while planning care, they ensure compliance with Federal and State regulations.

Special Feature/Requirements (license, certificates, etc.):
Valid Wisconsin Registered Nurse license. Must be able to demonstrate knowledge of state and federal regulations related to the RAI process. Clinical experience in long term care setting and show proficiency in MDS 3.0. AANAC MDS certification preferred.

MDS Coordinator Skills:
MDS Coordinators are experienced registered nurses with extensive knowledge in MDS. They are detail-oriented leaders who are capable of directing other people and managing schedules. Must be an excellent communicator and able to work cooperatively as a member of team. MDS Coordinators need to be flexible and decisive, responding quickly in high-pressure situations and completing work in a timely, accurate manner.

Essential Functions:
Makes clinical decisions to guide the effective practice of geriatric nursing utilizing physician direction and nursing judgement. Performs all duties in accordance with organization’s policies/procedures, state and federal regulations and in conformance with recognized standards while performing the duties identified below

• MDS Administration
• Assess the health status of residents and communicate the information with appropriate members of the health care team.
• Utilize the nursing process, nursing and medical practices and procedures, following laws and regulations that pertain to long term care to meet resident needs.
• Completion and/or review of all MDS assessments prior to submission to ensure accuracy.
• Oversee the assessment process, including setting the assessment schedules and assuring that assessments are done in an accurate and timely manner.
• Transmit assessments to CMS.
• Assist facility nursing administration in developing and updating the care plan
• Identify significant change in conditions and manage appropriately
• Assist in determining Medicare skilled level of care
• Assist in direct resident care such as handling or moving residents.
• Participates in implementation of QAPI programs for nursing
• Educator
• Educate C.N.A.s on ADL scoring and other MDS-related content.
• Educate members of the IDT on MDS coding changes, impact on reimbursement and RAI/MDS regulations and compliance
• IDT
• Communicate with the organizations’ nursing leadership to enhance our MDS process
• Promote organization’s team and servant leadership concepts.
