We are looking for a Family Nurse Practitioner with expertise in outpatient care. You will be responsible for providing primary health care and will use your skills to diagnose and treat patients. The ideal candidate will have hands-on experience educating clients on their conditions and medications, as well as an understanding of the health practices of the community.
Responsibilities:
- Quickly assess patients' health problems and provide care.
- Manage and track patient care.
- Communicate with physicians., managers, community resources, family members., and nursing staff.
- Counsel patients on healthy lifestyle choices.
