This Opportunity:
The primary responsibilities of the Family Advocate, Child ACTT, will be to participate on a multi-disciplinary team that serves children in their natural setting. The Family Advocate, Child ACTT, will support families who need assistance with navigating multiple systems and services. The Family Advocate, Child ACTT will assist in identifying needed resources for families and will provide linkage with both basic and more complex needs
What You'll Do:
  • Attend and provide input into the Child and Family Team monthly meetings on each child served
  • Link to public support services (e.g. DSS, transportation, financial assistance, food and medication) as needed
  • Meet regularly to review the total caseload, problem-solve, develop plans, and determine daily duty assignments.
  • Attend and actively participate in meetings and training as required. Maintain certification in all agency, state and federal training requirements.
  • Documents participant activities consistent with the service records manual. Write an event note that reflects participant activities, goals, interventions, and outcomes.
  • Act as liaison with the program after hours staff on a daily basis through exchange of participant information; briefly detailing the presenting problem and after hours plans for the individuals being served by the Child Assertive Community Treatment Team.
  • Meet regularly with the other Assertive Community Treatment Team members to formulate and disseminate information and be available by telephone and/or pager.
  • Provide skill-building to individuals who are served by the Assertive Community Treatment Team.
  • Facilitate training of independent living skills for both children and their families, establish access to community services and agencies, assist individuals in establishing support networks in the community and assess individual needs on a daily basis.
  • Attend daily team meetings
  • Maintain trainings as required and requested
  • Demonstrate knowledge of emergency procedures and assist in crisis situations
  • Demonstrate knowledge of and comply with all agency policies and procedures
  • Complete all other relevant responsibilities assigned by the supervisor
  • Driving and travel may be required

Education We're Looking For:
Bachelors, High School (Required)
Certifications We're Looking For:
CPSS - NC Division of Mental Health, Developmental Disabilities and Substance Abuse Services, North Carolina Driver License (Valid) - NC Division of Motor Vehicles
Experience We're Looking For:
Experience working with youth with Serious Emotional Disturbance (SED)
