DESCRIPTION OF POSITION:

This position provides transportation of patients to and from the facility providing continuous observation, interaction, and role modeling to the patients while maintaining safety at all times. Non-mechanical upkeep of van, painting and cleaning of patient rooms, offices, public areas and other assigned duties.

KEY RESPONSIBILITIES:

· Provide transportation for patients to and from the facility.

· Report all van maintenance problems to the Manager of Facility Operations.

· Responsible for the van log ensuring that the address and phone numbers are updated.

· Communicate and consult with the Charge Nurse (R.N.) and other staff members regarding patient care issues and information.

· Demonstrate responsibility and sound judgment regarding patient safety.

· Maintain a clean environment for patients at all times.

· Performs related duties as requested.

Minimum Requirements:

Education and Licensing:

· High School or equivalent required, plus technical training and experience.

Knowledge and Experience:

· Required license- Driver’s License

Skills and Abilities:

· Maintains confidentially of patients at all times.

· Sensitivity to, and willingness to interact with, persons of various social, cultural, economic and educational backgrounds.

· Address patients, visitors, and co-workers in a pleasant and respectful manner.

· Adhere to facility, department, corporate, personnel and standard policies and procedures.

· Attend all mandatory facility in services and staff development activities as scheduled.

· Adhere to facility standards concerning conduct, dress, attendance and punctuality.

· Assist in any emergencies that may arise.

· Assist in maintenance on an “as needed” basis.

Physical Requirements:

While performing the duties of this job, this position is frequently required to do the following:

· Use standards office equipment and access, input, and retrieve information from a computer. Use computer keyboard with manual and finger dexterity and wrist-finger speed sufficient to perform repetitive actions efficiently for extended periods of time.

· Communicate effectively in person or via telephone in a manner which can be understood by those with whom the person is speaking, including a diverse population.

· Give and follow verbal and written instructions with attention to detail and accuracy.

· Perform complex mental functions and basic arithmetic functions; interpret complex laws, regulations, and policies; collect, interpret, and/or analyze complex data and information.

· Vision: see details of objects at close range.

· Coordinate multiple tasks simultaneously.

· Reach forward, up, down, and to the side.

· Sit or stand for minimum periods of one hour at a time and come and go from the work area repeatedly throughout the day.

· Lift up to fifty (50) pounds.

Job Type: Full-time

Pay: $17.08 - $22.43 per hour
