We're looking for someone to join our team as a Director, Well-Being who:

  • Supports well-being initiatives in the community in addition to leading the employee strategy at market hospital, which is a microcosm of that community. Much of the work leverages our partnership with Blue Zones and operationalizes the research from the Blue Zones communities around the world where people lived demonstrably longer with the best health. Supports the strategic vision for individual, organizational and community well-being, with programs that include: Blue Zones�; Blue Zones� Campus and Associate Well-Being - health promotion framework that leverages Blue Zones research by operationalizing evidence-based practices through environmental design, culture change, strong social connections and aligned policies through a personalized campus action plan called a blueprint; Blue Zones� Community Projects - a pioneer in taking a systemic and environmental approach to improving the well-being of entire cities and communities; and Cal-AIM - a Medi-Cal program that allows health systems to integrate more seamlessly with community-based social services, leveraging community partnerships to improve health outcomes, reduce inappropriate utilization, and improve well-being and quality of life for assigned individuals in the community. Leverages local community benefit budgets and grant funding to ideate, implement, or sponsor, community-based interventions as determined by local leadership and community identified health needs. Serves as the local account manager and ambassador for Adventist Health well-being priorities, liaison, well-being partner to the local executives, leveraging the expertise and systems to support an impactful market well-being strategy.

    Essential Functions:

    • Facilitates and manages well-being strategy, acting as local liaison and account manager from the Adventist Health system well-being team. Provides direction and leadership of well-being initiatives in the market, leveraging Blue Zones, CalAIM and/or social determinants of health (SDoH) best practices. Influences and advises hospital-based strategies that incorporate a knowledge of all well-being strategies outlined above. Facilitates input from multi-disciplinary, internal and external stakeholders (Well-Being Committee) to create a comprehensive well-being playbook for local market. Facilitates end-to-end data sourcing and tracking, management and reporting of both program-specific and system-identified key performance indicators (KPIs) and measures representing each well-being strategy. Reports accountability measures to the system well-being team and local well-being committee on a quarterly basis, for all strategies implemented locally.
    • Serves as the local coordinator during the Blue Zones Campus Certification Foundation/Discovery period. Leverages the support and technical assistance of the associate well-being and Blue Zones campus certification teams. Supports the local Adventist Health executive sponsor related to program implementation. Leads the implementation of the Blue Zones Campus Certification Blueprint (aka action plan) with associated documentation and tracking. Acts as primary liaison to the Blue Zones team during the implementation process. Act as advisor to the Blue Zones team related to organization dynamics and history as requested. Participates in training on the Blue Zones campus certification model and associated activities as requested. Organizes and manages the internal Blue Zones campus certification program oversight and implementation steering committee, blueprint workstreams, and ambassador network. Provides training to these committees as requested.
    • Leads other Adventist Health associate well-being program elements, such incentive programs, biometric screenings, workshops, and local initiatives. Partners with campus leadership (such as mission and human performance) to align strategies, create synergy, and increase impact. Compiles and tracks identified metrics in support of the implementation process. Serves as the local coordinator during the Blue Zones campus certification review process. Collaborates with the Blue Zones team related to evolution and continuous improvement of the worksite model. Develops and implements strategic opportunities for broad employee engagement in the community and increases intentionality of community health to help make our community work an Adventist Health competitive advantage.
    • Implements a comprehensive community health strategy that supports the system and local community strategy, vision, values, and goals. Coordinates, collaborates and communicates with leadership, market well-being committee and the community governing board in the development and execution of the community health strategy. Serves as the local coordinator, advisor and primary liaison to CalAIM Enhanced Care Management team, local market and AH system. Supports CalAIM community supports strategy, serving as community liaison. Supports funding and outcomes reporting initiatives related to CalAIM strategies and programs. Identifies and aligns strategy with existing and emerging funding models and sources to ensure sustainability of initiatives. Represents organization's community health interests with external parties and develops partnerships with community resources.
    • Facilitates collaboration and alignment with population health management initiatives, including implementation of strategies to support defined populations and development of community capabilities, systems and processes to manage super utilizers. You'll be successful with the following qualifications:

Education:

  • Bachelor's degree in life sciences, health science, health promotion, or healthcare related field or equivalent combination of education/related experience: Required
  • Master's Degree: Preferred

Work Experience:

  • Seven years related experience: Preferred
  • Three years of experience in developing and implementing worksite wellness programs, community-based social determinants of health (SDoH) interventions, and/or community-based patient case management: Preferred
